HCOAudit – Data Survey
Your HCOAudit will be calculated by collecting 10 data points for each current employee.
Instructions:
Please complete the following 10 fields for each full-time employee relevant to the scope of your audit. For instance – if you are solely doing an audit for your software development team, there is no need to add other roles outside of this.
Once you have completed the data survey, make sure to hit the “submit button”.
The following fields below are defined as follows:
Position
List the title or role of each employee
State or Province
List the state or province for the location of each employee
Base Salary
Provide the base salary of each employee
Bonus %
List the target annual bonus % for each employee (if no bonus is provided, please place a “0”).
Benefits Expense
Use this simplified formula to estimate the benefits expense per employee:
Benefits Expense per Employee=Total Benefits Expense÷Total Number of Employees.
Computer Expense
Use this simplified formula to estimate the computer expense per employee:
Computer Expense per Employee=Total Computer Expense÷Total Number of Employees.
Software Expense
Use this simplified formula to estimate the software expense per employee:
Software Expense per Employee=Total Software Expense÷Total Number of Employees.
Supplies Expense
Use this simplified formula to estimate the supplies expense per employee:
Supplies Expense per Employee=Total Supplies Expense÷Total Number of Employees.
Rent Expense
Use this simplified formula to estimate the rent expense per employee:
Rent Expense per Employee=Total Rent Expense÷Total Number of Employees
Equity Participation
